Step 1: Get a Dividend Miles account

Step 2: Fill in the pertinent information
Click on “Join Dividend Miles” on the top right of the web page. It will take you to a page with this form on it:
All you have to do is fill in all the pertinent information in the given spaces on the form. Once you’re done, click one “Continue”.
Step 3: Select the items you want delivered
You will now be taken to this page:
Select all of the items you want delivered to your email address, then move on to the bottom portion of the form. Type in your hometown airport in the space given on the left, and on the right type in the cities you go to the most. When you’re done, click on “Continue”.
Step 4: Check your information
You should now be seeing a page that looks something like this:
All you have to do here is make sure that all the information you put in is correct, in case of typos or mistakes. If everything is in order, type in the captcha on the bottom left and then click on “Submit”.
Step 5: Click the Apple Now button
Done? Congratulations, you have successfully created a Dividend Miles Account! Now, you should see an “Apply Now” button. Click on it and it will take you to this web page:
Once you’ve filled in all of the relevant information into the form, your application will be received by US Airways and they will contact you!
